Use this form to request an appointment with us. While we will do our best to accommodate your requested day and time, any appointment requests via email will not be confirmed until contacted and spoken with a member of our staff. 

For first time patients and clients we do require the exam fee to be pre-paid upon booking your first exam.

**If you fail to make your appointment  or do not reschedule your appointment within 24 hours of your exam, you will have to prepay for your next exam fee**
